Please complete all the information required. When entering your name please ensure it is the same as shown on your passport.

 B. Home Institute

Please tell us where you are currently studying. Please also provide the details of a representative from your home institution; this is so we can resolve any issues that may arise before your arrival.

C. Study Details

Please indicate which teaching block you wish to attend at Portsmouth Business School and choose your units from the relevant Curriculum Choice list.

D. Signature

All forms must be signed and stamped by the institution at which you are studying. If applications are not signed then the form will not be deemed valid and disregarded. Please ensure that this form is completed and returned with your application.
